INSTRUCTIONS AND FORMS FOR MEDICAL SERVICES

Welcome to the Carolina Campus of the University of Puerto Rico!

In compliance with the security and control measures imposed by the Department of Health in light of the COVID-19 pandemic, we have digitally facilitated the delivery of the documents required by the Medical Services Office. For this process to be effective, it is important that you read the specific instructions for each document before uploading it to portal.upr.edu.

Each time a document is received or reviewed, the system will send a message to your institutional email address as proof that the process has been completed.

IMPORTANT

  • The documents to be sent must be originals and in PDF format, unless otherwise specified.
  • Employees of the University of Puerto Rico authorized to study in our system, under current regulations must comply with the requirements of this form.
  • The official documents to complete are in blue, when you click on them you will find a link to them.

New students, professional improvement, transfers, transfer, readmission and special permission:

Students Under 21 Years Old

  • Upload through the UPR portal in PDF format the original vaccination certificate (PVAC-3/Green Sheet) before the enrollment process.
  • Additionally, you must bring the original document to the Medical Services Office the first week of classes.
  • The certificate must include the COVID-19 vaccinations. (at least 2 doses)
  • This document must be completed on a computer and cannot have erasures or handwriting that invalidates it.
  • Exceptions to vaccination due to health conditions must be accompanied by a medical certificate. The certificate must state the specific reason and possible duration of the condition or circumstance that contraindicates immunization.
  • Students who are not vaccinated must send a notarized document specifying the reasons for non-immunization.
  • Special Permit Students do not require the delivery of the original document to the Medical Services office.
  • Upload to the UPR portal in PDF format the original result of the CHEST X-RAY READING signed by a radiologist.
  • Upload to the UPR portal in PDF format the original result of the TUBERCULIN TEST in mm signed and with the license number of the authorized person, with less than one year from the date of registration.
  • Students with Special Permission do not require the delivery of the CHEST X-RAY READING or TUBERCULIN TEST document.
  • Upload to the UPR portal in PDF format the MEDICAL CARE AFFIDAVIT document signed and notarized before a lawyer.
  • Additionally, you must bring the original document to the Medical Services Office the first week of classes.
  • Special Permit Students do not require the delivery of the original document to the Medical Services office.
  • If you have a medical plan, upload to the UPR portal in PDF format certification of validity from the insurance company indicating the validity of your plan, it must provide the expiration date of your coverage.
  • If you do not have a medical plan, please fill out the Student Medical Plan Information Register to apply for the student medical plan and upload it to the UPR portal in PDF format.
  • The University of Puerto Rico, in contract with a recognized insurance firm, provides its students with a medical plan of free selection at a moderate cost.
  • The student who, at the time of enrollment, does not have a private or government medical plan, must take advantage of said policy.
  • The policy is effective beginning the first day of classes after having paid the premium corresponding to the selected insurance and ends the day before the beginning of the next academic section.

Students Over 21 Years Old

    • Upload the original vaccination certificate (PVAC-2) through the UPR portal in PDF format before the enrollment process.
    • Additionally, you must bring the original document to the Medical Services Office the first week of classes.
    • The certificate must include the COVID-19 vaccinations.
    • This document must be completed on a computer and cannot have erasures or handwriting that invalidates it.

International Students

  1. Student must meet all the above requirements that apply according to their age.
  2. Physical Examination: Upload to the UPR portal in PDF format the PHYSICAL EXAMINATION document signed by a doctor and with his license number less than six months from the date of registration.
